  2. print, complete, and mail in the advisor approval form (PDF)
  3. print, complete, and mail in the assumption of risk form (PDF)
  4. mail in your official college transcripts 
  5. mail in a $40 application fee

PLEASE NOTE: Failure to answer these questions will delay processing of your application.  If your answer to any of the following is "yes," you must submit a full statement of relevant facts.  You may be required to furnish the college with copies of all official documentation explaining the final disposition of the proceedings.

I understand that I am applying for admission as a Winter at Westminster Visiting Student, and any credit I may earn as such will not be considered nor applied toward a degree at Westminster, unless and until I complete requirements for admission as a degree-seeking student.  I am allowed to enroll in a maximum of 16 semester hours of coursework as a Winter at Westminster Visiting Undergraduate Student.

I understand that withholding information requested in this application or giving false information may make me ineligible for admission or continuation at Westminster College.
